About Nexus:
The Southeast Regional Crisis Center (SERCC) is a new 24/7 walk-in mental health facility designed specifically for people experiencing a mental health crisis. The center provides a calm, safe and welcoming environment. Patients receive immediate help from compassionate staff who are experts in crisis, trauma, and mental health care.
SERCC is open 24 hours a day, everyday, to people of all ages in the 10-county region, regardless of their financial situation or insurance status. We help to stabilize their immediate crisis, and develop integrated plans for ongoing care they may need when they leave the center.
SERCC also houses short-term residential facilities for those that need longer care. With 16 beds, the center has separate units for adults and youth (ages 10-18), has nursing services, individual and group counseling, and care coordination upon discharge.
Position Summary:
Our Case Manager - Community-Based Services coordinates ongoing therapeutic services and community support necessary to ensure clients can receive stabilization care.
Primary responsibilities:
Client Consultation & Treatment Planning
Assists clients in linking to services, identifying barriers, and connecting with supports prior to discharge Provides consultation and support to families, youth, and adults in need of services Work closely with the teams to create stabilization plans, discharge plans, and supported engagement Develops treatment plans and utilize a variety of treatment approaches with clients Provides assistance to families regarding resources available to the families and youth that will support the healthy functioning of the family unit.; serves as a resource to families for consultation and crisis intervention Screens direct referrals for needs, assist in triaging individuals Completes intake screening and assessments as necessaryService Delivery
Works effectively with outside organizations to successfully implement plans for adults and children entering crisis services Works to bridge relationships with adults, families, youth, and outside organizations and provide role modeling for advocating for needs Provides timely and accurate communication to supervisory and management staff and team members regarding significant events, to ensure continuity of care and safety of clients Create aftercare plans and follow upOutside Organization Collaboration & Support
Communicates with families, legal systems, schools, mental health providers, probation, and social service agencies; works effectively with outside organizations to successfully implement plans for adults and children Work to bridge relationships with adults, families, youth, and outside organizations and provide role modeling for advocating for needs Maintains database of community resources and contacts for outside providers and organizations; collects data on services, linkages and additional outcomes as identified Completes the aftercare process of discharge planning, follow up on connections to outside services and conduct satisfaction services for providers and individuals receiving servicesQualifications:
Bachelor’s degree and/OR 5+ years of experience in social work, case management or similar duties as outline Required to meet MN Mental Health Practitioner definition as defined in the MN statutes/section 245.462, subdivision 17 located at https://www.revisor.mn.gov/statutes/cite/245.462 Valid driver’s license required. Must meet state regulating agency and Nexus Home Office driving requirementsNice to have:
Bachelor’s Degree in Human Services 2+ years of experience managing a caseload related to youth/familiesLocation and pay:
