Case Manager 12th Street
Position is eligible for a $1250 sign on bonus! Unity House of Troy’s Transitional Apartment Services Program (TAS) 12th Street has openings for a full time Case Managers. Position is Monday - Friday days. Responsibilities: Manage day-to-day routines and daily resident care coordination. Oversight of medication supervision room and medication inventory. Engaging in ongoing, regular interactions and providing supportive services to program consumers. Assist consumers with severe and persistent mental illness. Maintaining charts and documents in keeping with Office of Mental Health guidelines. Assisting with daily living skills including light to moderate cleaning as needed. Communicating with providers on a regular basis. Rotation of on-call duties. Promote a culture of belonging and inclusion. Requirements: High School Diploma and at least two or more years working with people with mental health issues. Experience working with multiple populations preferred. Valid and clean NYS driver's license. Reliable transportation.
