Summary:  The Case manager works closely with the Clinical Supervisor, members of the  team and corrections staff to ensure seamless, integrated behavioral health needs during incarceration and upon release. The Case Manager works to ensure adherence to the National Commission on Correctional Health Care and may provide coverage with adults incarcerated and/or court involved youth.

 

Principal Duties and Responsibilities: 

 

Case Manager’s may provide the following for eligible individuals incarcerated or court involved: 

Function as a member of the Individual’s jail transition team. Identify Individual-perceived barriers to discharge, assist the Individual with working through barriers and assure the Individual that they will be supported throughout the process. Coordinating in conjunction with release planning efforts for the individual to travel back to his or her community. Facilitate a “warm hand-off’’ to the behavioral health agency chosen by the Individual. Warm hand-off activities may include: Helping the Individual complete any necessary paperwork for receiving Behavioral Health services, recovery services, accessing community resources.Supporting the Individual’s self-advocacy in the development of their own community treatment plan and treatment activities

 

Professional Development and Quality –  

 

Monitor all ethical and safety concerns and report them immediately;Maintain full and open communication with the BHRS Clinical Supervisor, promoting communication and information between the team members, and other agency staff in delivery of services;Ensure that service provides continuous quality and understanding of the national standards for the National Commission of Correctional Health Care that addresses both client needs and compliance with WACs, RCWs, RSN, Medicaid, and Federal Regulations;Ensure compliance with all applicable Lifeline Connections policies and contracts;Other job duties as assigned.

 

EDUCATION, EXPERIENCE, AND KNOWLEDGE REQUIRED BY THE POSITION

 

Bachelor’s level education in a behavioral health field requiredMaster’s level education and ability to meet MHP qualifications as defined by the WAC and RCW preferred.Individual must obtain and maintain licensure from the WA Department of Health.Have strong case management skills and demonstrated experience to ensure robust engagement with people who have criminogenic attitudes. Having skills and competence to establish supportive trusting relationships with persons with SMI or SED.Respect for patient rights and personal preferences in treatment.Demonstrated experience at providing a safe and supportive environment for individuals with SMI or SED.Valid Driver’s license for the state of Washington.

 

COMPLEXITY

 

The incumbent provides agency based services to patients in re-entry services.  Strong communication skills, sound judgment, and creativity are required to help implement and deliver support services that meet the complex needs of the patients.

 

PERSONAL CONTACTS

 

Contacts are with members of the jail/juvenile detention staff, probation staff, patients, significant others of the patients, administrative, supervisory and clerical personnel, employers and representatives of various community agencies specializing in the treatment of substance use and mental health. All personal contacts are carried out in accordance with federal and state statues, laws and regulations dealing with the confidentiality of alcohol and drug dependent client records.

 

PHYSICAL DEMANDS

 

While performing the essential functions of the job, the employee is regularly required to sit, speak clearly, hear and use hands and fingers to manipulate writing utensils and keys on a keyboard.  The employee is occasionally required to stand, walk, reach with hands or arms, stoop, kneel, crouch or lift and/or move maximum of 15 lbs.

 

WORKING ENVIRONMENT

 

Working hours are spent indoors in the jail or juvenile detention facility, offices or meeting rooms and contact in the community meeting with patients in their homes, health clinics or other community agencies.  Occasional visits to community agencies, and participation in staff retreats and staff development activities may be required.

Salary: $23.98 - 26.36/hr DOE
