Pivot Point Youth Center provides short-term residential treatment to girls (ages 12 to 18) with various challenges including maladaptive behavior patterns, moderate mental health disorders, emotional and/or behavioral disturbances; whom may have a history of trauma, substance use disorders, and long-term involvement within the Department of Juvenile Justice Services. Our team embodies dedication and sincere passion to providing quality treatment, education, 24-hour supervision, a series of applicable skill sets, thorough services, and a consistent, safe home-like setting predictable through routine. We are seeking a Full-Time Case Manager to join our efforts as we believe this is more than a job, but a meaningful career supported by a likeminded company essential to true change in the lives of community adolescence.
Schedule: Tuesday - Saturday, 9am - 6pm
What you'll do:
A Case Manager is responsible for administrating, developing and facilitating the completion of case plans and treatment programs in accordance with the requests of the placing agency and the needs of each student on their case load. The Case Manager reports to the Program Manager. Responsibilities include:
Develops and oversees the completion of the MAP Plan according to the Discrete Case Plan, Commitment Time, and presenting issues and needs of the students on the assigned case load. Provides monthly progress notes and weekly student updates to the client manager and/or placing agency official according to the Discrete Case Plan and a student’s progress toward the MAP Plan. Adheres to the facility and daily schedules. Facilitation of Weekly Group Meetings. Conducts assigned responsibilities with respect, consideration, encouragement, positive guidance, and utilization of resources towards the youths’ overall success. Other duties as assigned, verbally or in written form.
To be considered, you should:
Be empathetic patient and treat others with respect. Have a BA/BS in a related field. Have prior case management experience, including 3 years with youth and/or juvenile placement agencies. Have a desire to work with troubled teens. Be able to pass a criminal background check, drug screen, physical, and TB test.
