Case Manager

Pay $19.00 - $22.00 an hour

The CCBHC Case Manager (CM) works within the Community Health and Behavioral Services (CHBS) program and is responsible for addressing Social Determinants of Health (SDOH) needs of individuals presenting to CHBS. The CM assists individuals in attaining stability across all life domains; providing referrals, advocacy & accompaniment, and supports individuals with the development of rehabilitative interventions and identification of resources to reduce future stressors.

 

CMs provide comprehensive Targeted Case Management (TCM) services and collaborative, hands-on Psychiatric Rehabilitation Services (PRS) to clients who are having problems due to crisis, barriers to obtaining services and/or in need of skill development to strengthen personal and community life. The CM works closely with all members of the CHBS multi-disciplinary team and maintains a monthly caseload of individuals and families.

 

Core Responsibilities

Coordinate care across settings and providers to ensure seamless transitions for clients across the full spectrum of health services.  Conduct TCM & PRS intake assessments to develop a person-centered and recovery-oriented Treatment Plan. Provide advocacy, translation, psychoeducation, skill development, accompaniment and linkage to care for all the services that will help clients increase personal and family stability and achieve recovery goals. Collaborate with all multi-disciplinary team members to maximize integration of care across all systems and programs. Provide comprehensive hands-on training to enhance coping skills, social skills, and daily living skills. Use Motivational Interviewing (MI), Screening Brief Intervention and Referral to Treatment (SBIRT) and brief counseling to address barriers to achievement of recovery and case management goals. Provide rapid intensive intervention to homeless individuals with a suspected or documented mental illness/substance use problem and assist them in developing and maintaining stability in their lives. Complete all required documentation according to program regulations. Participate in agency or community activities as part of the treatment team that may include meetings, trainings, committees and community events. Engage in outreach efforts to promote awareness of CHBS services.

 

Qualifications

High School Diploma or GED required and 1-3 years of relevant work experience in a behavioral health setting OR a Bachelor’s Degree. Must complete required training (s) within three (3) months of hire date and not to exceed six (6) months of employment. Excellent interpersonal skills and able to communicate both verbally and in written form. Commitment to cultural diversity and sensitivity. Valid NYS Drivers license and ability and willingness to travel in the served community to assist individuals in their home and other community settings.
