Essential Functions of the position are detailed below, and include any physical requirements below that. * Accomplishes clients' care by providing brief case management and supportive services to clients and/or their families in order to promote self-sufficiency and in coordination with management and/or the regional director and volunteers.* Assesses if clients are eligible for financial assistance, accurately and expediently processes the necessary documentation to process their applications and follows up, when necessary.* Maintains up-to-date and accurate departmental files and databases as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies in the course of performing job expectations.* Responsible for maintaining, updating and submitting routine and ad-hoc reports.* Participates in community outreach efforts and must maintain a flexible schedule to attend community events, committee meetings and special events, as needed, and must be willing to travel between sites to fill in for staff.
