Case Manager

Overview

QUALIFICATIONS: Bachelor’s Degree in Social Work, Psychology or Counseling from an accredited college or university and/or bachelor’s degree from an accredited college or university with 3 or more years’ experience in the human service field. Requires a working knowledge of child welfare services, experience working with children and/or adolescents, experience in psychological assessment and counseling approaches.


SUPERVISION: Is responsible to and evaluated by the Student Services Supervisor, who in turn, reports to the Treatment Directors.


POSITION PHILOSOPHY: Functions as an extension of the clinical team to provide assistance with service planning including outreach, follow up, maintaining student’s comprehensive clinical record, coordination of care, discharge planning and aftercare follow up.


ESSENTIAL FUNCTIONS LISTED BUT NOT LIMITED TO:
1. Protects student rights and represents interest of students in overall matters. Treats students with dignity and respect.
2. Functions as a mandated reporter and reports any suspected student abuse, whether physical sexual or mental/psychological to Quality Assurance Specialist
3. Successfully completes orientation period
4. Coordinates admission and discharge of students per assigned caseload, working with Treatment directors, Admission Directors, and other T. C. Harris team members.
a. Initiates and coordinates referrals to other agencies, BDDS services, etc.
b. Exchanges necessary information and uses Release of Information consent forms
5. Establishes communication with referring agents and parents/guardians for new student within the first week of admission.
6. Reviews with student and or parent/guardian policies for student’s related to setting up home visits, phone contact, belongings, etc.
