Luxor Nursing and Rehabilitation at Mills Pond is seeking a Case Manager. Long-term care experience preferred. Candidates must be a current NY Licensed Practical Nurse (LPN).

The Case Manager Assists residents and the Nursing Facility's Interdisciplinary Team to achieve residents’ goals of a successful rehab stay and safe discharge back to the community. Case Manager will be responsible to assess treatment needs; develop, monitor, and evaluate treatment plans and progress; facilitating interdisciplinary approaches; monitoring resident progress performance and act as the key clinical & social liaison with resident family and/or external parties.

Responsibilities include but not limited to:

Collaborate with hospital to provide updates and progress on mutual patientsComplete discharge from facility/ re-integration back to communityScheduling appointments post dischargeSending medication list to physician’s post discharge to ensure continuity of careMeeting with families to explain rehab process and expectationsWorking with admissions, clinical, rehab & social services staffAttending care plan meetingsPre discharge education to residentsSome charting required LPN certification required
