We are seeking a Case Manager for our Case Management Program to join our team in our Dover, DE location.
Position Details
This role entails conducting case sessions mainly within community settings, including client homes, schools, and other local venues. Case management duties may occur in either an office or community environment, depending on case requirements. Flexibility is crucial, as occasional evening or weekend work may be necessary to meet client needs. The role also involves potential on-call availability to provide consistent support and responsiveness.
Coordinates resources and provides essential information to meet client needs, ensuring all resource coordination is documented accurately and efficiently. Manages client discharge referrals, actively engaging families in creating a comprehensive discharge plan that includes options for service providers and community resources. Oversees record-keeping, compliance, and authorized unit tracking to ensure client charts are up-to-date, including incident reports filed within 24 hours as necessary. Evaluates and documents safety conditions in resource homes to ensure a secure environment. Completes case documentation in compliance with Merakey standards, county MHDS/CASSP, OMHSAS, and other regulatory and licensing bodies.Earn: $20.67 per hr.
