Case Manager

Review donor chart information and medical records to ensure that the donors meet the criteria set forth by MTF, and to provide a completed, accurate donor chart for the Donor Eligibility determination and release for processing by the Medical Director. **Responsibilities** 1. Perform post recovery clinical review of the donor record. The donor record includes critical recovery documentation, medical social history, plasma dilution, relevant communicable disease (serology) test results, authorization for tissue donation, relevant medical records, autopsy, reports, and any other pertinent information submitted or communicated by the Recovery Partner and other facility that impacts donor eligibility. 2. Identification, evaluation, and coordination of required corrections for discrepancies and/or omissions that impact donor eligibility. 3. Review charts for regulatory compliance with MTF policies, industry standards and all applicable regulatory guidelines. 4. Track and monitor the production of Fresh Tissue and Special grafts to ensure a high priority initial review, reconciliation of discrepancies and Medical Director review within the time sensitive requirements. 5. Interaction and communication, verbal or written, with Recovery Partners to obtain relevant information or required corrections that impact donor eligibility. 6. Provide a complete, comprehensive donor chart to the MTF Medical Director for evaluation, Donor Eligibility determination and tissue release authorization. 7. Review and evaluate weekly tracking and information reports to ensure timely process flow in the review of donor charts. 8. Daily interaction and collaboration with Team Case Coordinator to coordinate and ensure timely process flow in the review of donor charts. 9. Interaction with NOCC team regarding screening issues or with other MTF department when necessary (Planning, Quality Assurance, Donor Log-In, etc.) as a resource or to obtain relevant information. 10. Interaction with other Donor Services workgroups to discuss program issues, problems, and successes. 1.
