Case Manager
Description We are offering a contract opportunity as a Case Manager. You will play a critical role in our operations, managing various responsibilities from processing client information to coordinating with community partners. Responsibilities: • Accurately and efficiently handle client referrals and appointments. • Prepare necessary intake paperwork and collect client identifying documentation. • Provide initial program orientation and outline of emergency shelter program expectations to clients. • Develop individual case management plans collaboratively with each client, focusing on financial planning and housing goals. • Accurately and timely input client data into the Homeless Management Information System (HMIS) and other data management systems. • Conduct needs assessments, make appropriate community referrals, and provide support to connect clients to ancillary services. • Assist clients with housing needs, including searching housing leads and providing landlords with insights on services/programs. • Help clients with employment, transportation, childcare, and counseling related needs or concerns by connecting them to appropriate community resources and referrals. • Ensure client compliance with program policies and expectations, providing warnings when necessary. • Maintain detailed case files, including information about clients, program expectations, client progress notes, and documentation. • Collaborate and communicate with other service providers, community partners, resident managers, and landlords/property managers as it relates to individual work or client needs. • Conduct yearly follow-ups with former clients to gain feedback and housing status. • Maintain an understanding of Fair Housing Laws and keep current on legal issues and regulations.
