Case Manager

Are you interested in working for a dynamic organization that is invested in their clients and staff?     The Case Manager will report to the Clinical Supervisor and in collaboration with other members of the clinical team, is responsible for providing the necessary case management and support for our clients. The Case Manager will provide a range of services, including case management, assessment, service planning, referrals, crisis intervention and guidance in using available resources to assist families in meeting service goals.  Responsibilities: Serve as a liaison for the clinical team, the client, the family, and all collateral service providers  Produce and maintain treatment materials and binders  Document all actions taken in connection with a case in a client’s medical record  Meet regularly with treatment team and caregivers to ensure proper implementation of the treatment plan, address specific target areas, review overall progress toward goals, and discuss changes to the treatment plan if needed  Ensure compliance, accuracy and quality of documentation and clinical records   Ensure compliance with Federal, State and agency guidelines related to administrative and clinical policies  Ensure clients receive quality and professional services   Respond to parent/caregiver/family concerns and determine appropriate resolutions  Participate in Clinical Team Meeting and other meetings as assigned  Required Experience: Bachelor’s Degree with a major in social work, counseling, psychology or related human services field  Two years minimum experience working with children, youth and families in a home or community setting preferred  Experience working with children diagnosed with behavioral-emotional disorders or children with Autism Spectrum Disorder  Valid driver’s license with reliable and consistent access to a vehicle  Benefits: (Full time employees)  Health, dental & vision  3 weeks paid vacation  11 paid holidays   (including Juneteenth, the day after Thanksgiving, Christmas Eve, and NY Eve.)  Generous sick time including mental health days  401k retirement plan with up to 4% match  Voluntary short and long-term disability  Voluntary Life Insurance  Dependent care savings account  Mileage reimbursement  Longevity award - Monetary bonus and a vacation day annually  Company provided laptop and cell phone  Opportunity for professional growth  Supervision for licensure provided  Tuition Reimbursement   Personal cell phone discount   Comp ensation Salary
