How can you make a difference in people’s lives?
We are seeking a Case Manager who is fluent in one or more of BHSD’s five threshold languages: Spanish, Chinese, Vietnamese, and Tagalog. The ideal candidate will have a passion for serving diverse communities, excellent communication skills, and the ability to deliver culturally competent services.
In this role, you’ll make a difference in the lives of people with substance use challenges, and often with co-occurring conditions. You’ll use and grow your skills in compassionate care, crisis intervention and de-escalation, in a supportive and inclusive work environment. You’ll have the opportunity to help people who are just discovering possibilities in their journey, going from a place of hopelessness to a place where they are valued and have the potential for a better life.
Principal Duties include but are not limited to the following:
Provide clients with a single point of contact for support addressing multiple health, social services and legal systems
• Advocate for clients engaging in services within the community
• Create a safe space for clients and maintain appropriate boundaries
• Be flexible, community-based, and client-oriented
• Assist the clients with needs of substance use and mental health as well as assistance thought to be outside the realm of substance use and mental health treatment
• Able to learn and apply various models and theories of addiction and mental health and other problems related to substance use
• Ability to describe and practice the philosophies, practices, policies, and outcomes of the most generally accepted and scientifically supported models of treatment, recovery, relapse prevention, and continuing case management for addiction, mental health, and other related problems
• Organize client transport to their appointments
• Transport clients to appointments when necessary
• Provide support in Milieu when needed
• Crisis management and de-escalation
What will make you successful in this role?
+ Certification with Substance Use state certifying body in California (CAADE/CCAPP/CADTP)
+ Experience working with customers or clients that feel overwhelmed or in a crisis (crisis intervention).
+ Experience working with clients who have Substance Use disorders.
+ Extensive experience with case management of underserved populations in a public setting.
+ Experience working with diverse populations and cultures.
+ Experience working with customers or clients with co-occurring disorders and disabilities.
+ Familiar with customer service principles.
+ Experience with resolution of conflict in stressful situations.
What else is required?
+ Valid California driver’s license with a good driving record and car insurance.
+ Current CPR/First Aid certificates are desirable .
+ Fingerprinting/drug testing required at time of hire.
+ Preferred: Bilingual in English, Spanish, Chinese, Vietnamese, or Tagalog
