Case Manager Job Details Job Location North Highlands - 4616 Roseville Road - North Highlands, CA Position Type Full Time Education Level High School Salary Range $24.15 - $24.15 Hourly Job Category Nonprofit - Social Services Description POSITION SUMMARY: Person in this position will be responsible for the case management of a large caseload of participants in the STOP Program. Essential Job Functions include those listed below. ESSENTIAL JOB FUNCTIONS: + Case management of a large caseload of participants released from prison substance abuse programs. + Heavy documentation. + Coordination of various services throughout several agencies. + Travel to Community Based Providers in personal vehicle. + In-prison presentations. + Parole collaboration. + Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and + Perform all other duties as assigned. Qualifications ESSENTIAL QUALIFICATIONS: To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Experience and Competencies: + Knowledge of the criminal justice system. + Excellent language and interpersonal skills. + Superior organizational skills and attention to detail. + Basic computer skills. + Good documentation skills. + Professional appearance and demeanor. + Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required. + Must be able to obtain and maintain CDCR Clearance. + CPR Certification, First Aid Certification, and an annual Tuberculosis test. EDUCATION and/or EXPERIENCE: + Minimum two (2) years’ experience working with the criminal justice population. + One (1) year case management experience is preferred. + Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology, or a related field is preferred. + High School diploma or its equivalent is required. LICENSURE/CERTIFICATION: + Not applicable for this position. WORKING CONDITIONS: + Work is performed primarily in an office, residential, or outpatient setting. + Regional travel is required.
