Case Manager Rochester, NY (http://maps.google.com/maps?q=265+N.+Clinton+Ave.+Rochester+NY+USA+14605) Description Status: Full-Time Reports to: Executive Vice President of Programs Job Summary The Urban League of Rochester seeks a full time Case Manager, who will play a critical role in ensuring the League achieves its goals, mission, and vision in relation to providing access to resources desperately needed by senior members of community seeking the ability to age in place. This individual will report to the Executive Vice President and will be responsible for the day-to-day support and service delivery for our EISEP Program. This position provides an outstanding opportunity for a team member to make their mark and provide support to a vulnerable yet valued sector of community. Candidates must possess a passion for people, health initiatives and support services while providing a proactive client centered experience to help create an environment where our clients and families achieve success. Job Responsibilities + Provides case management services to functionally impaired elderly. + Screens individuals for program eligibility. + Conducts in-home psychosocial assessments to determine client needs and financial status with client and caregiver. + Completes full assessments on the computer. + Develops and implements comprehensive care plans. + Authorizes and arranges for home care through contracted Aide Agencies/Day Programs. + Authorizes payment of bills by tracking services. + Monitors clients and services in accordance with state regulations. + Develop and implement appropriate discharge plans. + Keeps up-to-date records and documents according to policy. + Acquires knowledge of EISEP regulations and standards. + Keeps updated on new regulations and standards set by State Office for the Aging. + Acquires up-to-date knowledge of aging services and professional practices through reading and attending seminars, workshops, and conferences. + Participates in staff meetings and case reviews. + Completes all statistics and written reports in a thorough and timely fashion. + Conducts outreach and public relations activities as requested. + Awareness of and active support of the Agency's Mission, Vision, Values, and Strategic Plan. Requirements REQUIRED: ·Credentials:Valid NYS Driver’s license with reliable transportation. Bilingual in Spanish preferred. New York State online Case Manager Certificate preferred or willingness to complete training course within first four months of hire. ·Physical Demands/Work Environment:The physical demands/work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. PREFERRED: ·Education:Bachelor’s Degree in Human Services or related field OR Registered Nurse with one year experience in the field of Aging. (BSW preferred). ·Experience:2 years in Human Services, preferably in the field of Aging.
