Job Description
The Case Manager will work alongside a family physician to coordinate and carry out various initiatives and communications. This role involves conducting office visits, collaborating closely with providers and staff, performing follow-up outreach, and educating patients both in person and through electronic means.

Pay and BenefitsThe pay range for this position is $48.00 - $52.00/hr.
