Job Brief

Case Manager set clear, measurable goals in an effort to work towards housing permanency.

Shift Worked: 9:00AM - 5:00PM (Mon - Fri) Weekly Hours: 37.5 FLSA Status: Non Exempt Pay range: $40,000-$45,000

POSITION OVERVIEW: 

Case Manager set clear, measurable goals in an effort to work towards housing permanency. Case Manager empowers residents to achieve their goals by creating a collaborative and supportive working relationship with clients based on empathy, accountability and trust. Case Manager provides information, referrals, and coordinate services such as: health care, job training, housing and immigration services among others. Case Manager is responsible for developing strategies for engaging with residents and creating awareness about services in the community and developing a path to re-housing.

Job Responsibilities:

• Case managers carries a caseload of 25 residents

*Ensure that everyone assigned is personally interviewed, assessed, within 24 hours of admission • Conduct intakes and makes assessments of resident’s needs and coordinate referrals as needed • Along with resident, develop individualized service plan with short and long term goals • Monitor compliance with goals through regular weekly scheduled appointments. ILP are reviewed with the resident on a bi-weekly basis

*Maintain file in an orderly fashion and up to date. Missing appts must be documented and remedy action to make up the appt as well • Work closely with shelter staff to enforce rules and regulations for residents

* Hold conferences with the residents when rules are broken or disregarded • Serve as an advocate with agencies and service providers to minimize barriers to residents receiving needed services • Follow-up with referral sources to ensure that clients receive, and access necessary services in a timely manner. Accompanied a resident to an appt when strictly necessary

• Coach clients in job prep, housing availability, social services and other areas as needed • Represent CP Unlimited at city and statewide coalitions, task forces and committees as assigned by supervisor • Participate in trainings and meetings, case conferences and training seminars as indicated by Director of Social Services

*Comply with all DHS Absorb trainings as scheduled by DHS

* Perform other duties as assigned by the Case Work Supervisor

 

Job requirements 

Bachelor’s degree or some college in Human Services or related field.  Minimum of one year counseling experience or homeless services Demonstrated sensitivity and experience with and/or knowledge immigration and disenfranchised communities including individuals whom identify as lesbian, gay, bisexual, transgender, queer, and/or HIV-affected, and individuals with mental health issues  Must have excellent written and oral communication skills, Able to work under pressure, able to respond to a crisis situation, health or safety issue and take appropriate and immediate action • Must be able to work flexible hours including evening and weekends • Must be computer-literate and must have knowledge of Microsoft Office applicationsCPR/First Aid RequiredNarcan Certification required

*Ability to work flexible schedule including evenings and weekends
