Provide assessment and service planning to address OUD and identified social determinates of health Develop, implement, and monitor service plan consistent with identified needs Communicate effective and in a timely manner with persons served and collaterals Attend external and internal meetings relevant to individual served and/or services Ensure that persons served progress notes, service plans, and other required documentation is completed professionally and accurately and in the electronic health record in a timely manner Ensure that consultation on the person’s served behalf is conducted Provide wraparound care and serve as primary point of contact for individual served Attend relevant trainings as required by Program Director
