 BayCare Health Systems is currently seeking an experienced Case Manager to join our outstanding and compassionate team in Winter Haven! This is a field-based position that works throughout all of Polk County.

 The Case Manager is responsible for:

Providing linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.Coordinating the service delivery system for patients.Providing residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.Medical Care CoordinationPerforming other duties as assigned.

Flexibility and adaptability are also essential for this role, given the ever-changing nature of our work and the need to respond to changing client needs and program requirements.

 Required Education:

Bachelor's in Related Field Preferred Bachelor's in Psychology, Social Work,  Human Services or a related Mental Health Field 

 Experience:

1 year in Mental Health required

 Required Certification/License:

BLS (Basic Life Support) Driver's License
