Performs concurrent and retrospective review of patient medical records for purposes of utilization review, care coordination compliance with requirements of external review agencies including government and non-governmental payers and quality assurance. Promotes a cooperative and supportive relationship as liaison with patient, family, facility staff, physician; funding representatives and community agencies. Ensure continuity in the handoff of patient clinical information from the hospital to other involved healthcare entities. **What qualifications you will need:** **EXPERIENCE REQUIRED** + **2 - 5 years Case Management Experience within the last year** + **Other required experience: Minimum of at least two years of RN experience.** **EDUCATION REQUIRED** + **Associate’s Degree-Major: Nursing** + **Graduate of a nursing program: associate degree or baccalaureate program.** **LICENSES AND CERTIFICATIONS** **Required:** + **Current license to practice professional nursing in the state of Texas or Compact Nursing License.** + **If your compact license is from a state besides Texas, you will be required to obtain a TX Nursing License within 90 days after your start date.
