How often will this team member be working remotely? Hybrid
**Responsibilities - Case Manager - BH:**
+ Provide linkage, monitoring, planning and advocacy for the most appropriate mental health and community medical and social services.
+ Coordinate the service delivery system for patients.
+ Provide residents, faculty and community mental health practitioners with up-to-date available referral resources to provide continuity of care.
+ Perform other duties as assigned.
**Qualifications:**
+ **Required:** 1 year - Social Work
+ Or, 1 year - Case Management
**Education:**
+ **Required:** **Bachelors - Social Work**
+ **Or, Bachelors - Counseling**
+ **Or, Bachelors - Human Services**
+ **Or, Bachelors - Psychology**
