We are seeking a compassionate and dedicated professional to support our diverse patient population in accessing comprehensive healthcare services. As a Case Manager, your responsibilities will include: + Conduct comprehensive assessments of patients' needs, creating individualized care plans. + Collaborate with healthcare providers and community resources to ensure a holistic approach to patient care. + Advocate for patients' rights and coordinate access to LGBTQ-affirming services. + Facilitate support groups and educational sessions on relevant health topics. + Maintain accurate and confidential case records, ensuring compliance with HIPAA regulations. + Provide crisis intervention and emotional support to patients facing challenging situations. + Collaborate with multidisciplinary teams to enhance patient outcomes. You must be available for early morning, evening, and/or weekend shifts.
