Case Manager Coordinator

We’re looking for a dynamic and compassionate Case Manager/Family Advocate Supervisor to join our team. This unique, full-time role combines leadership and hands-on case management, making it ideal for someone who thrives in both supervisory and direct service capacities. Position Overview: Case Manager/ Family Advocate Supervisor, you will split your time equally between supervising a team of 4-5 Family Advocate/Case Managers across various schools and providing direct case management services. In this role, you will support and guide your team members, ensuring they deliver effective, high-quality services to families. You'll monitor their work, provide ongoing coaching and training, and ensure the successful implementation of program contracts. Your leadership will help shape the success of your team, and your work will make a significant difference in the lives of families navigating complex challenges. In direct service portion of the role, you will promote the healthy development and positive outcomes of youth and their families through case management and family education strategies, such as conducting assessments, resource linking, family workshops and advocacy in a Richmond elementary school. The Family Advocate intervenes in a wide spectrum of health and resource needs as part of the school’s Community School effort. The Case Manager Coordinator will report to the Healthy Communities Industry Director. Minimum Requirements: + At least 3 years Case management experience required. + B.A. Degree highly desired + Passionate about family advocacy and community-centered work, with a focus on equity and empowerment. + Experience in case management, social work, or a related field, with a track record of working with families in a multicultural, diverse, urban school community. + Experience working in collaboration with parents, teachers and school staff. + Experience leading workshops and support groups. + Strong communication and organizational skills, allowing you to balance the demands of leadership and direct service. + Knowledge of social, emotional, health and economic issues faced by West Contra County youth and their families. + Cultural humility and a demonstrated commitment to equity and social justice. + Ability to maintain goals and priorities in dealing with varying challenges. + Ability to be flexible in working with people and organizations with different viewpoints Preferred: + Spanish language ability highly desired. + Supervisory experience desired. Responsibilities: Supervision and Program Coordination: + Manage the team of 4 – 5 Family Advocates to ensure effective and impactful services to youth and families at numerous schools. + Provide individual and group supervision to support their work. + Develop and implement processes for referrals and provision of case management services. + Provide coaching and training, engaging addition resources as needed. + Build and maintain strong relationships with school staff, community partners, and other stakeholders to advocate for families and enhance service delivery. + Support the completion of documentation and reporting required by funders. Student and Family Services: + Provide trauma-informed case management services to youth and families including: conduct risk and needs assessment to guide the delivery of services, service plans, referrals and follow-up. Monitor progress, and track referrals to support positive outcomes. Transition planning as needed. The services will include individual meetings, family meetings, home visits, family appointments and collateral contacts. + Link families with needed health, mental health and other resources in the community. Assist families in overcoming obstacles to accessing services. Advocate on behalf of families. + Create and lead parent/guardian education workshops and/or support groups on topics related to family-school connections, healthy parenting, conflict resolution and child development and other trauma-informed strategies. + Provide crisis intervention services as needed. + Provide consultation with school staff the bounds of confidentiality. + Maintain accurate and up-to-date clinical records, statistical reports and other program documentation as required for program evaluation. School and Community Services: + Work closely with school staff, Community School Coordinator, youth therapist and parents to deliver services. + Participate in CARE Team and other student assessment processes. + Be familiar with community resources + Participate in Community School and agency meetings as required, such as project planning, supervision, and trainings.
