DESCRIPTION: Community Support Services is a clinical case management program serving individuals diagnosed with mental illness, who live independently in the community. Clients live in either Agency owned housing or their own residences. Case Managers work with the individuals on their personal goals in order to meet their needs and enhance their quality of life.\n

Qualifications:\n\nBachelor's degree in human services related field required\nExperience working with individuals who have mental illness \nExperience working with electronic health record system and Medicaid reimbursement programs a plus\nAbility to pass criminal\/registry background checks\nSuccessful completion and passing drug screen\nClear tuberculosis screening\nA valid driver's license, no more than 5 points or DUI in last 3 years\n\n

ESSENTIAL DUTIES AND RESPONSIBILITIES:\n\nProvide in home and on-site services and support as per each individual's treatment plan; including provision of direct service to clients in areas such as motivational interviewing, money management skills, daily living skills, symptom improvement, psycho-education, housing support, job skills training, etc.\nEvaluate skills and needs of each individual and make recommendations to treatment team\nLearn the 8 dimensions of wellness and multiple interventions in order to provide quality Medicaid services \nUtilize an electronic health record (EHR) system to write clear and concise progress notes to bill for Medicaid reimbursable services \nProvide crisis intervention services, as necessary, and crises prevention plans in an effort to improve crises response \nPass all Agency and program specific trainings\n\n
TRAINING:\n\nAll employees are required to complete 10-15 trainings upon hire and on an ongoing basis\nTrainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting\nAll trainings are paid\n\n
