Case Manager (CV42-2) - Homeless Services Division We are looking for a Case Manager to join our dynamic and growing team. The successful candidate will report to the Director/Treatment Coordinator and will be a vital part of their Treatment Team. JOB RESPONSIBILITIES: + Use advanced tools to conduct risk/needs assessments and develop service plans for all clients assigned to your caseload. + Implement those plans subject to review by the Treatment Team and/or approval of the Director/Treatment Coordinator. + Schedule and facilitate case management meetings with clients + Facilitate scheduled workshops, programs, and groups addressing clients' needs. + Maintain appropriate contact with outside agencies contracted to provide specific outsourced services. REQUIREMENTS: + Strong interpersonal skills + Work well in a team. + The capacity to achieve an effective balance between set operational policies and procedures and the use of initiative as and when the need arises + Possess effective written and verbal communication skills + Have an ability to interact appropriately with clientele + Basic computer skills will be preferred + Must have a valid driver's license and meet VOADV agency standards EDUCATION: + Minimum of a Bachelor's Degree Only those applicants with a minimum of a Bachelor's Degree should apply.
