Correctional Case Manager Position (Social Worker) - Dayshift
40 hours a week

Nature of Work inthis Class
This positionprovides direct service to residents, maintains case records and supervisesresident activities. The Case Manager isdirectly responsible for initiating, implementing and documenting individualand group treatment/habilitation services as they relate to the resident’sindividualized treatment plan objectives. Work is supervised by their Immediate Supervisor.
EssentialFunctions
· Manages all aspects of individualcaseload and individual case records and program records of residents assignedto them.
· Meet with Residentas required, and provides necessary communication to keep residentupdated on their progress.
· Techers class andlarge groups on facility curriculum.
· Provides dischargeplanning services to the resident to ensure success.
· Provides assessment of residents’needs/risks to determine treatment plans.
· Performs intake procedures for newresidents, which involves familiarizing these residents with rules andregulations of the facility.
· Apply behavior management system tomanage residents’ behaviors.
· Performs crisis intervention services.
· Prepares written reports.
· Attends staff meetings and other relatedmeetings as required.
· Coordinates and implements individual andgroup counseling services to residents.
· Develops and maintains effective workingrelationships with various treatment facilities and with the general public.
· Makes court appearances as designated.
· Maintains strict confidentiality withrespect to information obtained and processed in the operation of the facility.
· Conducts home and/or employment visits ifneeded.
· Transports residents to communityresources as needed.
Marginal Functions
· Supplements facility coverage – subjectto being called into work at any time due to emergency situations and/orcoverage needs.
· Utilizes audiovisual equipment.
· Performs other related duties asrequired.

· Knowledge of principlesof effective intervention including risk, need and responsivity.
· Knowledge ofcrisis intervention techniques.
· Knowledge ofsubstance abuse evaluation and treatment.
· Knowledge of localtreatment programs.
· Ability toestablish and maintain effective working relationships with residents,residents’ families, co-workers, outside agencies, etc.
· Ability to gather,collate and classify information about data, people or things.
· Ability to workunder stressful and demanding conditions.
· Ability toeffectively manage time and organize workload.
· Skill in oral andwritten communication.
· Ability to workindependently.
Working Conditions
Moderate noise inCorrectional Facility setting.
Physical Demands
· Work involves asignificant amount of standing, walking, sitting, talking, listening, stooping,crouching and reaching with hands and arms.
· Must be able totransfer up to 40 pounds.
· Ability to reachinto file drawers in standard four-drawer filing cabinets.
· Visual abilitysufficient to effectively operate a copier, facsimile machine and a computerterminal, and to read reports, correspondence and instructions.
· Hearing abilitysufficient to converse with other individuals, both in person and by telephone.
· Speaking abilitysufficient to communicate effectively with other individuals in person and bytelephone.
· Ability to readand prepare reports with recommendations.
