Case Manager
EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Case Manager to work full time for its Home Base Community program.
The Case Manager position is a non-exempt position paying $25 per hour at 35 hour. The hours of operation are Monday-Friday 9:00AM-5:00PM and the office is located in Central Islip, NY. The role reports to the Program Director.

Primary Purpose of Job:
The Case Manager is responsible for all aspects of Home-based Crisis Intervention and case management services for client families in the Home Base (Community)Program.
Principal Duties & Responsibilities:
+ Assist families in resolving the crises and problems that led to the youths involvement with court and/or Department of Probation. Provide case management support to families in crisis struggling with problems such as substance abuse and mental illness.
+ Provide intensive, wrap around home-based case management and crisis intervention services to help the family reach stability.
+ Conduct assessments using the Global Appraisal of Individual Needs (Gain) instrument and develop a Service Plan based on family needs, using a strengths-based approach.
+ Make referrals to partnership agencies and outreach to new agencies when required.
+ Follow-up with service providers to monitor families progress.
+ Assist parents and youth in learning behavior management techniques.
+ Maintain progress notes and accurate case records in computer system with all necessary data entry in accordance with contract mandates.
+ Transport and escort clients from appointments or to obtain emergency resources such as clothing or medication as needed in own vehicle when necessary.
+ Participate in team meetings, case conferences and trainings.
+ Facilitate and develop monthly educational workshops for adolescents when assigned by Director.
+ Participate in monthly recreational events for the adolescents in the program when needed.
+ Hold weekly female and male adolescent groups when assigned by Director.
+ Maintain progress notes and accurate case records with all necessary data entry in a timely fashion in accordance with contract mandates.
Knowledge, Skills, & Abilities Required:
+ Bachelors Degree required and one year of child and adolescent experience required or at least one-year community or social service work of child and adolescent experience.
+ Participation in at least 20 hours of training in the first six months of employment (with an additional 20 hours during the second six months of employment) on Homebuilders, mediation, working with adolescents, substance abuse, mental health, the juvenile and/or criminal justice system, and related topics. Training will be provided by outside trainers, EAC NETWORK and by partnering County agencies.
+ Must have good communication skills, both oral and written.
+ Must have access to automobile and possess valid New York State drivers license.
+ Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.
+ Bilingual (Spanish speaking) Preferred.
