We prefer one year of experience in Med/Surg, ER, or ICU to ensure our patients receive the best care possible. Education/License: Diploma from an accredited school/college of nursing. Required professional licensure at time of hire. Responsibilities: Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.| Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.| Carries out physician and nursing orders. Attends team meetings and care plan meetings.| Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.| Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.
