Case Manager-(HRA) SS Bronx Job Details Job Location Bronx, NY Position Type Full Time Education Level 2 Year Degree Salary Range $42000.00 - $45000.00 Salary Travel Percentage Up to 25% Job Shift Day Job Category Nonprofit - Social Services Description The Case Manager’s main responsibility is to provide case management services for a caseload of 22 clients and their family members. The Case Manager prepares comprehensive assessments and service plans. Home visits and Field work are required.The positions offer a salary range of $45,000-$50,000, with a generous benefit package including health insurance, dental, vision, dependent care, paid time off, 401(k), tuition assistance program and more. There are available positions in the Bronx and Brooklyn. Schedule: Mon.-Fri. 9am-5pm Responsibilities: + Ensure that each client is receiving appropriate and effective case management services + Conduct comprehensive assessments and develop comprehensive service plans that identify mutually agreed upon long and short-term goals. + Conduct face to face contacts (home visits/office visits) as specified in the service plan(s); with a minimum of one home visit per month and one contact per week. + Provide assistance in accessing primary medical care; assistance with adherence to medication regiments; teach activities for daily living; nutritional services; recreation programs; assistance in employment assessments, training, and private sector placement services; prevention with positives; and other necessary services to clients to increase their capacity for independent living. + Review the case management service plans and conduct reassessments on a quarterly basis to ensure that services are appropriate for the clients’ current needs. + Provide crisis intervention to clients and their families as needed. + Maintain ongoing awareness of clients’ medical conditions so that services are appropriate to changing needs. + Arrange support and home health care services as necessary, making referrals to medical, home care, mental health and/or substance abuse services + Ensure that clients pay the balance of their rent, telephone and utility bills. + Provide counseling and orientation regarding tenant’s rights and responsibilities to maintain and retain housing. + Assist clients in moving from hospitals, nursing homes or other locations into their apartments, and, provide transportation when necessary. + In collaboration with vocational counselor and entitlement specialist provide advocacy & referrals to employment programs for training and job placement in the private sector. + Assist clients in locating alternative housing options, when a client’s health so requires. + Provide and coordinate all supportive services as needed. + Collaborate with all providers within and outside of Comunilife involved with the client/family. Coordinate and collaborate with HASA personnel as required. + Maintain regular personal contact, developing a trusting working relationship with clients in order to monitor physical and emotional capacity for independent living + Maintain and complete required documentation including all contacts made with the client and any contact on their behalf + Participate in weekly supervision + Submit a weekly schedule of field visits + Participate in mandated training workshops, seminars, and case presentations as well as meetings with peers and supervisors + Perform any other duty as assigned Qualifications + Bachelor’s Degree in human services or related field with two (2) years’ case management work experience + Associates Degree in human services or related field with four (4) years’ case management work experience + High School Diploma with six (6) years’ case management work experience will be considered + Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) a plus + Knowledge of HRA Congregate Housing Program guidelines + Direct service experience with persons with living with HIV/AIDS, substance abuse, serious mental illness, and homelessness + Bilingual English/Spanish a plus + 5 Day/35 work week Non-Remote Position (In-Office/Some Field Work) + Excellent oral and written communication skills + Proficient in Google Workspace program/Microsoft Office Suite + Familiar with AWARDS, Child and Children Integrated Reporting System (CAIRS), Coordinated Assessment and Placement System (CAPS) Comunilife is an EEO employer and provides equal employment opportunities to all employees and applicants for employment and prohibits discrimination and harassment of any type without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
