Provide direct practice in the areas of group work, consumer education, treatment planning and individual counseling as needed. Participate as a member of the multidisciplinary treatment team, performing intakes, assessments, initiating treatment interventions and crisis intervention. Perform clinical case management and treatment planning with consumers that experience psychiatric disorders and substance abuse disorders. All clinical documentation is completed with compliance in the requirements set by the NJ Division of Mental Health Services and the Joint Commission on Accreditation of Health Care Organizations. Participates in staff training and continuing education. Takes an active role in Performance Improvement projects at EISS and MMC. Is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors, etc. Qualifications: Required: + BA/BS Degree + Three to five years clinical experience with the dual diagnosed target client population; Skills Training, Crisis Intervention, Group work, Individual and Family counseling skills are imperative. + Partial Hospital Program experience is also recommended. + Must possess and maintain a valid N.J. State Driver s License Scheduling Requirements: + Shift- Day- Hours are subject to change based on the program need + Monday-Saturday + Full Time Essential Functions: + Assists in patient billing through accurate record keeping of client s attendance in groups and activities daily + Assume active participation in Performance Improvement Projects. Initiate improvement efforts and work collaboratively with the treatment team to identify areas of needed improvement + Attend Inservice training to improve skill level and knowledge base. + Participate in training preparation and planning process when required. Support Department s inservice and continuing education committee + Attends and participates in multidisciplinary staff meetings daily and is prepared to discuss clinical and program issues + Complete Treatment Plan with direct participation from each consumer on caseload. Attend treatment planning meetings and conduct treatment strategy with other members of the treatment team + Complete documentation in the medical record according to department policy. Psychosocial Assessments, Group Notes, Progress Notes and treatment summaries include all required information and meet the time frame requirements according to policy + Facilitate Family Psycho-educational Program. Provide information and support group for family members of adult partial hospital program clients + Facilitate Patient Education through individual counseling and group work. Prepare education materials for distribution and presentation to mental health consumers + Facilitate Treatment Groups as scheduled. Lead mutual aid support groups and skills training activities. + Perform group work techniques and interventions to achieve individual goals for each group member + Facilitate individual and Family Counseling according to treatment plan recommendations. + Facilitate referrals to other services agencies and assist clients in obtaining appropriate entitlements (Social Security Benefits, Residential program, Self Help) + Function as liaison with inpatient unit specifically completing assigned consults within 48 hours and attending alternate treatment team meetings with the inpatient. unit on Tuesday mornings + Participate in the departments QA/PI efforts. Complete QA/PI monitors and follow-up on action plans + Provide Recommendations for improvement efforts + Perform Intake Assessments following initial contact or referral + Present Assessment to treatment team and complete the required documentation for clinical record + Perform crisis Intervention with the cooperation of other treatment team members. Perform interventions with the Screening center Staff when required + Performs intakes on assigned referrals.
