Job Overview:
The Case Manager facilitates treatment and provide direct practice in the areas of group work, consumer education, treatment planning and individual counseling, coordination of referrals and intakes as necessary.

Required:
Bachelor's degree in behavioral health/social service field1 year behavioral health case management experienceSkills Training, Crisis Intervention, Group work, Individual and Family counseling skills.Perferred:

Facilitate treatment and provide direct practice in the areas of group work, consumer education, treatment planning, and coordination of referralsParticipate as a member of the multidisciplinary treatment team, performing intakes, assessments, initiating treatment interventions and crisis interventionPerform clinical case management and treatment planning with consumers that experience psychiatric disorders and substance abuse disordersParticipates in staff training and continuing educationTakes an active role in Performance Improvement projectsCoordination of referrals and intakes from PESS, IP, PHP, State Psychiatric Hospitals, Community and other hospital systems including but not limited to scheduling and keeping track of the status of these referralsKeeping statistics on the outcomes of these referralsFacilitate referrals to other services agencies and assist clients in obtaining appropriate entitlements Social Security Benefits, Residential program, Self HelpBenefits and Perks:
