Adecco Medical & Science is hiring a Licensed Practical Nurse (LPN) in Kissimmee, Florida for a care management role in both inpatient and outpatient settings. This is an on-site opportunity. Please read the details below and apply with a current resume for consideration. **Type:** Three-month contract, with possibility of extension or becoming fulltime **Schedule:**  Monday to Friday, 8:00AM to 5:00PM **Location:** Kissimmee, FL - 34759 **Pay:** $30 to $32 **Responsibilities Include:** • Ensure continuity of care across inpatient and outpatient settings using appropriate resources within contract and benefit guidelines • Perform care management duties both on-site and via phone as needed • Act as the care team leader in collaboration with the PCP and care team • Meet with patients, families, and caregivers to discuss treatment and care plans under the Transitions of Care program • Identify and follow up with high-risk patients in acute, custodial, and ambulatory care settings • Collaborate with physicians and care team members to coordinate and implement treatment plans • Conduct patient assessments, triage, and treatments based on clinical orders • Complete biometric screenings and detailed clinical intake documentation • Facilitate telehealth visits: room patients, prep tech, and support provider workflow • Process refill, lab, and imaging requests per protocols and standing orders • Support clinic operations: front desk coverage, scheduling, and patient engagement • Review medical records for accuracy and completeness • Assist in delivering patient education during virtual visits on chronic care, disease prevention, and health maintenance • Monitor and track clinical orders and results • Contact patients with normal lab/imaging results per provider direction • Ensure accurate and timely EMR documentation • Provide clinical oversight and guidance to unlicensed assistive personnel • Apply logic and reasoning to resolve patient care issues and support care flow **Requirements Include:** • Active and valid LPN license in the state of Florida • IV Certification • Minimum 1+ year of experience in an acute clinical care setting • Background in care management, utilization review, or discharge planning • HMO experience preferred • Bilingual candidates strongly preferred **Why work for Adecco?** + Excellent Weekly Pay + 401(k) Plan + Skills Training + Excellent medical, dental, and vision benefits Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
