Case Manager (MAT) Job Details Job Location Dandridge - 207 W Main - Dandridge, TN Position Type Full Time Education Level 4 Year Degree Salary Range $22.00 - $24.00 Hourly Travel Percentage Up to 25% Job Shift Day Job Category Nonprofit - Social Services Description Position Summary: The Case Manager works to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants. Essential Job Functions include those listed below. Essential Job Functions: + Provides case management services and assistance to program participants struggling with substance abuse, and those in recovery; + Assesses the needs, conditions, and desires of assigned program participants; + Educates and refers program participants to community agencies and businesses that appropriately address their needs for food, shelter, clothing, employment, transportation, etc. + Assists participants with establishing or re-establishing community support systems; + Acts as advocate on behalf of program participants to improve access to vital services; + Participates in clinical staffing decisions regarding the participants current strengths and resources, and their need for additional resources; + Completes documentation of services provided in a timely manner; + Provides input into the patients treatment needs and disposition; + Conducts assessments of program participants and provides therapy as needed; + Reviews and documents all clinical services and activities in accordance with licensing standards and WestCare policies and procedures; + Participates in case conferences involving assigned program participants; + Assists other WestCare staff as needed to promote cooperation and portray a professional image to the community; + Embraces and embodies the mission, vision, guiding principles, clinical vision and goals of WestCare Foundation; + Performs any other duties as assigned. Qualifications Certifications/Licensures: + LPC, LCSW, or similar certification preferred;Preference will be given certified candidates; + Valid drivers license and the ability to obtain and maintain WestCares vehicle insurance is required. Education: + Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology or other field related to the position is preferred; + High School Diploma or equivalent is required; Experience and Competencies: + Experience working with individuals suffering from mental health and substance abuse disorders required; + At least two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred; + Ability to define problems, collect data, establish facts, and draw valid conclusions required; + Excellent communication skills are required; + Ability to exercise good judgment and discretion required; + Ability to work well in a team environment required ; + Adherence to the highest standard of ethical conduct, especially to standards governing confidentiality required; + Professional appearance and demeanor required; + Must be culturally/linguistically sensitive to populations served; + Intake and assessment experience is highly preferred; + Bilingual ability is preferred; + Prior experience using office equipment, including phones, scanners, printers, and fax machines, and web cameras is highly desired; Working Conditions: + Work is primarily performed in an office or other indoor setting; + Some outdoor activities and travel between multiple locationsmaybe required; + Regular and prompt attendance is required throughout employment.
