**Case Manager - Matching Grant** **Job Details** **Job Location** 87 North Clinton Ave - Rochester, NY **Position Type** Full Time **Education Level** Bachelors Degree **Salary Range** $20.00 - $21.50 Hourly **Job Shift** Day **Job Category** Family Services **Description** Under the general supervision of the Program Manager, the Matching Grant Case Manager is responsible for providing intensive case management and employment services to eligible participants of the Matching Grant program. Key responsibilities include: Identifying and enrolling eligible clients into the Matching Grant program. Offering support and employment services to help clients achieve self-sufficiency. Tracking and submitting data; completing and submitting monthly, quarterly, and annual reports to supervisors, funders, and partners. Coordinating with other staff, employers, community partners, volunteers, and service providers to connect clients to appropriate services for achieving self-sufficiency. **Essential Duties and Responsibilities** + Serves as the point person for both the client and the service providers, ensuring that the client’s individualized service plan is being implemented and provided. + Is responsive to client and community contacts, returning all phone calls as soon as possible but no later than the next business day **.** + Provides case management services beginning at enrollment and continues throughout the 240-day service period unless the case is no longer receiving services through MG. + Meets the funder mandates in person contact with prescribed documentation requirements at both intake, case closing and throughout the entire Match Grant service period. + Meets the Program and funder case management case contact expectation of a minimumof biweekly contact or communication effort, documenting progress of all case members, and follow up on tasks/activities. + Works with all adult case members to ensure their understanding of the expectations of the Match Grant around employment and potential consequences for not complying with program requirements. + Assists in resolution of barriers if present and impacting the case’s journey to self-sufficiency through early employment. + Completes all MG pre-enrollment activities by day 25 of 31 day of MG enrollment period. + Maintains case file with all required USCCB documentation. + Provides planning and coordination of service delivery with the MG job developer and program volunteers to ensure that all program requirements are met. + Case note documentation reflects the provision of all MG Program nine (9) required services provided by the program directly or through referral services and are provided in a timely manner, + Develops the individual Self-Sufficiency Plan appropriate to the client’s need and ensures that services provided are appropriate to the needs of the client. + Completes client budget/s at enrollment, day 180and day 240. An adjusted budget is completed at any time circumstances change. + Completes the Self Sufficiency Plan that includes employment goals and activities for all MG employable adults. + Serves as a mediator between the employer and employee when communication issues, or barriers to continued employment are raised. + Obtains all required financial documentation including proof of employment and associated required documentation reflecting the provision of MG direct assistance and cash allowance payments. + Ensures language services are provided for all case contacts and all documents requiring a client signature is translated in the language of the program’s primary caseload to ensure a complete understanding of the program and of the documentation that require their signature. + Attends all scheduled training opportunities. + Maintains outlook calendar, providing availability information for scheduling of case pre-enrollment/enrollment sessions and case contacts _Note: The above description is illustrative of tasks and responsibilities. Employees are required to follow appropriate supervisory direction and perform other related duties as required._ \#INSJ **Qualifications** **Education:** Bachelor’s degree in social work, Human Services, or related field. An equivalent combination of education and experience will be considered. **Credentials:** Valid and clean NYS Driver’s License and use of a registered and reliable vehicle. **Experience** : At least one year of experience in the Human Services field and/or case management experience. Experience working with volunteers, immigrants, and local service providers preferred. **Bilingual:** Bilingual in English / Spanish, Arabic, Dari, Swahili, or French preferred **Physical Demands/Work Environment:** The physical demands/work environment described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
