POSITION SUMMARY: The Case Manager LMSW is a professional who collaborates with physicians, health care team members and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness. Provides various social work services to meet needs of patients and family members. Services include psychosocial evaluation of patient and relationship to family members in considering patient medical needs and prognosis. Provides crisis intervention as necessary to patient and family members. Provides discharge planning for all patients as necessary to ensure a smooth transition. Social work practices and services are consistent with applicable accreditation and licensing standards, and reflect the National Association of Social Worker Code of Ethics.

What you will do:  

1. Performs social work services for patients found to be at risk including, but not limited to the diagnosis of substance abuse, psychiatric problems, domestic violence, pediatrics, end stage dementia, developmental disabilities and CVI. Completes psychosocial evaluation of patients and coordinate transition through the continuum of care.
2. Develops social work treatment plan and provides individual and/or family counseling as appropriate. Evaluates the socio-emotional status of patients and family members as necessary.
3. Provides various direct services including: crisis intervention, counseling, discharge planning, coordination of services post-hospital care, refers for financial assistance, makes referrals for community services as necessary to assist the patient in transitioning along the health care continuum.
4. Participates in Gallup survey. Participates in Impact planning sessions and the implementation of the Department specific Impact plan.

What you will need:  

1. Masters degree in social work required. Prior employment experience in a related area preferred.

2. Demonstrate ability and skill in the management of patient care planning and continuous quality improvement inservices delivered.

3. Recognized ability to be a key contributor as a member of a patient care team.

4. Well developed communication skills, both written and oral. Ability to communicate effectively with individuals and groups representing diverse perspectives.

5. State of Michigan, social work license required.

6. Ability to work flexible work schedule.

7. Strict adherence to patient confidentiality expectations.

8. Knowledge of applicable accrediting and regulatory standards.
