Discover a rewarding career at AJH, where you can make a meaningful impact within a close-knit community while advancing your professional growth within a thriving healthcare system._ **_Are you ready to become a part of our dynamic Case Management Team?_** Responsibilities include but are not limited to: + Appropriately identifies and assesses all new admissions for case management intervention based on interdisciplinary case referral and case management high risk criteria. Reviews all new admissions for clinical appropriateness based on severity of illness and intensity of service. + Develops an individual plan of care based on the needs of the patient and develops a realistic discharge plan in collaboration with the patient/family/health care team. + Implements a plan of care based on the needs of the patient, providing ongoing support and resources to patients and families, communicates effectively with payers’ changes in patient status. + Effectively communicates with the interdisciplinary health care team discharge needs and obstacles affecting treatment and discharge disposition. Makes referrals to appropriate agencies based on patient need and sets up family/resource meetings. + Prioritizes and manages daily case load based on patient and hospital need. Concurrently review’s patient treatment plan and progress to ensure that patients continue to meet level of care according to payer specific criteria and communicates this to individual payers in a timely manner. + Ensures that all patients are aware and in agreement with their individual discharge plan, reassessing and updating plans of care to meet patient and family needs, while advocating and respecting patient rights. + Complete, accurate and legible documentation follows mandatory documentation guidelines. + Maintains RN credentials, attends continuing education programs and completes annual ARK. + As a facilitator of the discharge plan is accountable and responsible for the utilization management, continuing evaluation and implementation of discharge planning in accordance with often changing individual patient needs. **Scheduled Hours:** 16 hr/wk, weekend/holiday rotation **Work Shift:** Day - 8a - 430p **Education:** Must be a graduate of an accredited nursing program. BSN Preferred. **Required Experience:** 3-4 years of relevant nursing experience in the acute care setting and previous case management experience, utilization review and/or discharge planning experience is required. **Required Licensures/Certifications:** Must be currently licensed as a Registered Nurse by the Commonwealth of Massachusetts.
