The University of Miami Health System, "UHealth", Department of Pediatrics- has an exciting opportunity for a Full-Time Case Manager - Peer Support Specialist to work in Miami.

The Case Manager - Peer Support Specialist coordinates the overall interdisciplinary plan of care for patients, from admission to discharge. The incumbent also provides ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient’s needs. An employee in this position acts as a liaison between patients, families, and healthcare personnel to ensure necessary care is provided promptly and effectively.

Core Responsibilities:                                            

Coordinates the integration of the social services function into patient care.

Assists patients in understanding their conditions and possible medical treatments.

Works with affiliated agencies and acts as an advocate between service recipients and these agencies.

Designs health plans for patients, which contain several activities from administering medication to consulting with other healthcare personnel.

Collaborates with other professionals to evaluate patients' medical or physical conditions and relevant needs.

Identifies environmental impediments to client or patient progress through interviews with patients and review of patient records.

Monitors, evaluates, and records client progress per measurable goals described in treatment and care plan.

Utilizes consultation data and social work experience to plan and coordinate patient care and rehabilitation, following through to ensure service efficacy.

Adheres to University and unit-level policies and procedures and safeguards University assets.

Department Specific Functions:

This role involves providing support and guidance to female clients who are pregnant, have children, are recovering from substance use, or have experienced mental health issues. The ideal candidate will have personal experience in these areas.

Provides supportive services to program participants

Assist individuals with linkage to behavioral health treatment and other community resources

Build supportive relationships with program participants, emphasizing the development of trust, mutual respect, encouragement, and emotional support

Help persons served identify and/or re-engage with family, friends, and other natural supports and other community resources

Ability to help set mutually beneficial boundaries with individuals 

Help the individual served identify strengths and recognize successes to promote self-esteem and empowerment.   

Reduce or eliminate barriers for consumers to attend medical appointments

Maintain appropriate standards of confidentiality and ethics

May have to travel using personal vehicle to different locations where the individual may be served including home visits.

Help individuals served identify goals and make choices that promote resiliency and facilitate recovery

Respond effectively to the most sensitive inquiries or complaints in a calm, professional manner

Validate the person’s served experiences and provide guidance and encouragement for the person to take responsibility and actively participate in their own recovery

Report clinical concerns and high-risk persons and safety issues as they occur to program staff and supervisors

Ensure that services reflect and support an individual’s differences, language and cultural diversity

Exhibit nonjudgmental approach, effective listening, good eye contact, and positive interactions

Follow up with individuals served via phone calls, home visits, and visits within the community.

Complete program documentation.

CORE QUALIFICATIONS:                                                                                                    

Bachelor’s degree in relevant field

Minimum 3 years of relevant experience

Personal experience with substance use, substance use recovery (minimum 2 years sober), or mental health challenges preferred.

Skill in completing assignments accurately and with attention to detail.

Ability to analyze, organize and prioritize work under pressure while meeting deadlines.

Ability to process and handle confidential information with discretion.

Ability to work evenings, nights, and weekends as necessary.
