Case Manager Registered Nurse | Home Health

Find your purpose in treating patients where they’re most comfortable – in their home. As a Home Health Case Manager Registered Nurse, you will work independently providing skilled care for 5 to 6 patients per day. Common duties include wound management, infusion therapy, assessments, education and training for patients on their new diagnosis, and more. We provide extensive training and ongoing RN office support while you’re making home visits to patients. This position will be working in Annandale, Maple Lake, Clearwater, Kimball and surrounding areas. 

Schedule:

Full-time | 72-80 hours every two weeks (10 scheduled days) Days | Monday-Friday | 8am-4:30pm  Every 4th weekend | Two holidays per year 

Pay and Benefits:

Pay based on experience and degree Associate/Diploma in nursing starting at $37.27 per hour Bachelor’s in nursing starting at $38.20 per hour Master’s in nursing starting at $39.16 per hour Pay range: $37.27 - $55.91 per hour Milage reimbursement and extra pay for working weekend shifts $3,500 hiring bonus for eligible candidates Full-time benefits: Medical, dental, PTO, retirement, employee discounts and more!

Qualifications:

Graduate from an accredited school of nursing required Current Minnesota Registered Nurse license required Current American Heart Association (AHA) BLS Healthcare Provider card required Minimum of one year of RN experience or 2 years experience in a Home Health setting   Reliable transportation required Valid Minnesota driver’s license, a driving record in good standing, and insurance liability coverage required

Based out of our office in Marketplace Mall, the Home Health department primarily takes place in the communities we serve, in the homes of our patients. Our staff provides services not just for St. Cloud but our surrounding areas as well including Buffalo, Alexandria, Little Falls, and more. Teams are assigned to areas to minimize driving.
