Position Summary: The Social Worker is responsible for the process of social service intervention of hospital inpatients, outpatients and patients whose cases are managed through the continuum of care in the community. Social Workers are also members of the Case Management Department, and partner with Case Managers on those patients who require a wider dimension of services and are part of collaborative efforts of a multi-disciplinary team to achieve disease-specific outcomes and objectives over the continuum of care while supporting the triple aim of improving the patient experience, improving patient outcomes, and effectively managing the cost of care and will completed other projects and duties as assigned while exhibiting exemplary core customer service values.
Essential Functions:
Manage Mental Health patient placement & Indigent patient needs in the Emergency Department.
Identify At-Risk Patients and expedite Complicated /Complex Discharge Planning on all In – Patient referrals.
Collaborate with the Interdisciplinary team to accomplish and document Resource rounds weekly on assigned units including discharge huddles
Complete all Readmission reviews within 48 hours excluding weekends
Qualifications
Education:
Bachelor’s Degree in Social Work or closely related field required
Licensure/certification:
Licensed Baccalaureate Social Worker in the state of SC required.
Experience:
Prior Case management experience in health care delivery acute care setting preferred.
Demonstrated knowledge and experience with Case Management and outcomes preferred.
Knowledge in acute care level psychosocial intervention and practices preferred.
Special Skills:
Able to work independently, adjust priorities often and establish rapport quickly required
Demonstrated experience in diplomacy and ability to contribute to team goal setting and program development required.
Proven communication skills, both verbal and written required.
Expertise in facilitation of dysfunctional family and patient placement outcomes preferred.
Basic computer knowledge and skills required to manage multiple databases required.
Primary home base will be established but flexibility to float to various facilities within the GHS system as may be needed required.

While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; frequently to use hands, fingers; and frequently to talk or hear. The employee must exert up to 15 pounds of force occasionally (activity or condition exists up to 1/3 of the time), and/or up to 5 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job and will be measured by post offer Physical Agility testing with a required “Light” rating to verify ability to meet the requirement.
