Under direct supervision, serves as a primary support and case manager for individuals with mental illness and/or co-occurring disorders by assisting them to live as independently as possible in their chosen community.
Shifts Available:
Hourly position at 40 hrs per week | Monday - Friday | 8:00am-4:30am
Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
What You Bring to the Table (Must Have)
High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services Must possess at least 1 year of experience providing services as a Substance Use Specialist Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver. To Make Referrals: The Vida ACT program has a closed referral process. All candidates must be referred by the Office of the Public Defender’s Defense Transition Unit (DTU) and the San Diego Sheriff’s Department. Referring parties must use the County of San Diego Single Point of Access System. All referrals must be screened and accepted before exiting custody and coordinated release to Vida ACT.

Who We Serve: Adults who have a serious mental illness, have a possible co-occurring substance use disorder, who are justice-involved, are eligible for probation, are on parole, and who may be homeless or at risk of being homeless.
Hours of Operation: Monday through Friday: 8:00 AM to 4:30 PM
About the Program: We believe that recovery starts from within and will do whatever it takes to support you on your recovery journey. We have a multidisciplinary team that includes a nurse practitioner, a program administrator, a licensed team lead, a peer recovery coach, mental health rehabilitation specialists, and case managers with specialties in housing, education, employment, and substance use who are all here to help you on your path.
