Job Summary

Responsible for completing intake interviews; determining need for medical and psycho-social evaluations; reviewing therapist and physician evaluations, care objectives, and plans; coordinating services being provided; advocating for needed services and entitlements.
Coordinating services being provided; arranging resources; observing and evaluating treatments and responses; advocating for needed services and entitlements; conducting interdisciplinary meetings and evaluations and assisting manager with the day-to-day operations of the department.

Essential Functions
-Determines approach by evaluating the physical, psychological, functional, environmental, and financial needs of patients. Determines psychosocial needs and the compound medical needs of patients.
-Defines care goals of patients by providing education, information, and direction to each individual and family
-Audit case file on a regular basis for completeness and accuracy.
-Assist in discharge planning as required
-Participates in case staffing and reports concerns
-Attends mandatory staff meetings and training sessions. Adheres to the mission, values, and health performance standards.
-Assists others and supports the team.
-Identifying gaps or barriers in treatment plans


 

Qualifications

Education
Bachelor's Degree Nursing required or Master's Degree Social Work required

Licenses and Credentials
Licensed Independent Clinical Social Worker [LICSW - State License] - Generic - HR Only preferred Registered Nurse [RN - State License] - Generic - HR Only preferred

Experience
Case Management, Utilization Review, or Discharge Planning experience 5-7 years required

Knowledge, Skills and Abilities
- Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
- Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems.
- Highly developed communications and interpersonal skills, working with varied population.
- High level of service delivery Demonstrate initiative with ability to prioritize work, meet deadlines and adapt to changing situations.
- Ability to establish strong rapport and relationships with patients and staff.
- Proficient in Microsoft Office and industry related software programs.
- Identifying complex problems and reviewing related information to develop and evaluate options and implement solutions.
- Ability to maintain client and staff confidentiality.
- Understanding of diagnostic criteria for dual conditions and the ability to conceptualize modalities and placement criteria within the continuum of care.
- Knowledge of Healthcare and Managed Care preferred.
