The CBHC (Community Behavioral Health Center) Registered Nurse will perform the following core functions: complete triage evaluations, administer injectable medication, engage in medication reconciliation, take vital signs, perform phlebotomy, coordinate medical care, contribute to comprehensive assessments, complete individual crisis prevention planning, provide psychoeducation, and assist with transition of care planning and care coordination.

Minimum Education Required Bachelor's Degree Additional Shift Details one weekend day a month, four 10 hour shifts Responsibilities Provide comprehensive nursing assessments and documentation. Collaborate with participants to develop and implement Wellness components of their Integrated Treatment Plans.Provide medical monitoring and screening of vital signs, lab work, substance and tobacco use. Assist in making sure all lab work required for medication monitoring is completed on time and that results are screened and flagged for provider review as needed.Identify priority health needs of individuals and the CBHC population.Complete standardized assessment scales such as the Clinical Opioid Withdrawal Scale (COWS), Clinical Institute Withdrawal Assessment for Alcohol (CIWA) and the Clinical Institute Withdrawal Assessment for Benzodiazepines (CIWA B). Work with participants, staff, family members, and other providers to effectively manage transitions in care.Administer long-acting injectable medications and engage in medication reconciliation.Participate in training on the CBHC’s identified Evidence-based practices (EBTs) and provide treatment consistent with these practices.Support individuals in participating in other CBHC services, E.g., Peer Support, Recovery Coaching, Care Management, Psychiatric Rehabilitation Services, Social Support Opportunities, and Community Recovery Supports.Coordinate service delivery with team members, including ensuring access and securing authorization for services.Collaborate with existing providers and other collaterals and deliver services in accordance with client’s plan of care.Participate in team meetings to ensure effective communication among all disciplines involved in participant’s care.Follow the CBHC participant across the continuum of care to ensure care coordination.Partner with participant, their care team providers, and supports to ensure that care plan and safety plan are implemented as developed and make needed adjustments.Monitor participant’s progress in treatment and assist the clinical team with evaluating the need for continued services.Complete all required documentation in a timely manner meeting Agency and division established deadlinesAttend assigned trainings; maintain necessary certifications and licenses (CPR/First Aid, Safety Care, MAP).Attend and participate in supervision, teaming and clinical rounds with medical team. Ensure that clients are treated with dignity and respect in accordance with Advocates Human Rights Policy. Adhere to all Advocates Way principles. Perform all duties in accordance with the agency’s policies and procedures.Follow agency Performance Standards. Qualifications Registered Nurse (RN) license.  Current Massachusetts registration in nursing with minimum of 3 years of direct work with adults in a community setting.Demonstrated knowledge base and judgment necessary for independent clinical decision making.Excellent verbal and written communications skills. Bi/trilingual (Spanish/Portuguese) preferred.Strong interpersonal and customer relations skills required. Excellent organizational, time management and problem-solving skills. Strong computer knowledge.Knowledge regarding psychiatric rehab and understanding of recovery model.Demonstrated understanding of and competence in serving culturally diverse populations.Commitment to Advocates values and mission.Must hold a valid drivers’ license.  Must have access to an operational, insured vehicle.Must successfully pass background checks
