CCS LPN
Schedule: Per diem, hours vary   Salary: $37.50 / hour   The Per Diem Licensed Practical Nurse (LPN) will provide direct service and daily nursing interventions in a Youth Community Crisis Stabilization (CCS) Program, designed to meet the needs of individuals ages13-18 who have behavioral health symptoms who require 24-hour per day care in an unlocked treatment setting.

The Per Diem LPN will provide nursing and medication administration services as well as provide health promotion training and access to medical care to clients and function as part of an interdisciplinary team and support individual's treatment goals using psychoeducation, crisis prevention planning and skills training.

The Per Diem LPN will help ensure the program meets high quality standards and provides value to individuals and a community care system while ensuring a safe and clean environment. The Per Diem LPN will liaison with multi-disciplinary collaborative and community providers as needed.   The Per Diem LPN also: Assists the YCCS RN with filling prescriber's orders, administering medications, and monitoring vital signs. Works with the YCCS Direct Service Workers in ensuring an environment that promotes safety, recovery, and treatment. Contributes to the assessment, treatment plan, individualized crisis planning, discharge planning and care coordination for each individual. Implements treatment plans using recovery-oriented, motivational interviewing, harm reduction, trauma-informed, culturally competent, and strengths-based rehabilitation
approaches to help youth successfully and safely transition to home or an alternate optimal
setting. Provides nursing care, teaching, and assistance with activities of daily living according to each client's plan. Takes and documents individual's vital signs, including temperature, pulse, respirations, and blood pressure per prescriber's orders. May include pulse oximetry. Uses crisis intervention techniques when necessary. Accesses on-call back up when needed. Completes incident documentation as needed. Performs other duties as required.
