Job Duties: + Oversight and compliance of program requirements + Interacting with Consumers and Attendants + Coordinating Consumer and Attendant On-boarding + Opening new Consumer cases in their home + Troubleshooting problems and providing resolution + Ability to handle a multi-line phone system + Highly Organized & Detail Oriented + Annual in-person visits to Consumers + Conduct monthly case management calls via phone + Payroll processing related duties Experience Needed: + Past experience in a medical field preferred + Previous Care Coordination or CDS experience is a plus + Medical office or healthcare experience preferred + Prior management experience preferred.
