Position TitleCertified Diabetes Educator - Part Time/20 hrs per weekEnglewood Plaza, Medical PavilionPosition Summary / Career Interest:This role is Part-time(20 hrs/week) working with 2 clinics/week.
The Diabetes Educator educates patients with diabetes on meal planning and general diabetes self-management education. They participate in the development and implementation of new/current nutrition and education programs. The Diabetes Educator assists in maintaining ADA accreditation and continuous quality improvement projects.Responsibilities and Essential Job FunctionsBe available in clinic most days of the week for additional one-on-one teaching as needed (e.g., instruction on insulin pen use, glucometers, insulin pump) Conduct DSME and/or nutrition clinics 4 days per week Coordinate, manage, and teach group diabetes education class Coordinate/implement other external events as appropriate Create and implement an annual Diabetes Day event Create and implement CQI projects (a requirement of ADA recognition) Assist the Education Coordinator with maintaining ADA recognition Help with coordination and teaching of other classes (e.g., Managing Diabetes and Pump Preview) Helps maintain ADA recognition and keep up to date Work closely with the administrative staff on scheduling and other matters Work with vendors for samples and updates Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department. Required Education and ExperienceBachelors Degree in an appropriate field (e.g., Nursing, Dietetics-Nutrition, or a biological science) from an accredited college or university. High School Graduate 2 or more years of clinical experience in diabetes and/or nutrition education.
Preferred Education and ExperienceMaster's Degree in an appropriate field (e.g., Nursing, Dietetics-Nutrition, or a biological science) from an accredited college or university. 2 or more years of clinical experience in an ADA or AADE accredited program.
Required Licensure and Certification CDE certification (NCBDE) required within 12 months of hire. Licensed Registered Nurse (LRN) - Single State - State Board of Nursing in state of practice. OR Registered Dietitian (RD) - Commission on Dietetic Registration (CDR) in state of practice. Basic Life Support Heartsaver (BLS HS) (Non-Clinical) - American Heart Association (AHA)
