Certified Family Peer Specialist

As a Certified Family Peer Specialist on the Integrated Health Care team, you will support client care by collaborating closely with Integration Specialists and Systems Navigators. You will provide non-clinical assistance to families, helping them achieve their goals while enhancing their child’s ability to function effectively at home, school, and in the community. This is accomplished by educating parents about available resources and guiding them through problem-solving strategies.

Qualifications:

Valid Certificate from the Minnesota State Colleges and Universities (MnSCU), demonstrating the completion of an approved community health worker curriculum or DHS approved “Certified Family Peer Specialist” or "Certified Peer Specialist" High school diploma, GED, or equivalent, required.  Must demonstrate strong competency in navigating systems of care within children's mental health. Ability to perform office-related duties as well as carry out responsibilities in home and community-based settings, including the ability to work off-site and travel as needed. Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice, internal self-awareness and reflection. 

Responsibilities:

Provide Comprehensive Care Management Work with the family in completing documentation for the intake process Supporting client’s individualized, holistic Health Action Plan with the client/client’s identified supports through appropriate linkages, referrals, coordination and follow-up to needed services and supports. Supports care coordination for children and families ensuring access to medical, behavioral health, pharmacology and support services. Develops caring relationships, builds trust with clients and families and promotes continuity of care. Identifies barriers to care and connects client/family with community resources and services. Researches, identifies and keeps up to date on available client/family resources within Fraser and the community on an ongoing basis. Demonstrates and applies knowledge of the philosophy/principles of comprehensive, community-based, family-centered, age-specific, and culturally-sensitive care coordination services. Health Promotion Services Encourages and supports healthy living concepts to motivate clients/families in managing their health and wellness. Support the Process of Referrals to Community and Social Support Services Accompanies clients/families to appointments as needed.

Administrative Responsibilities:

Complete documentation in a timely manner as outlined in the regulations and in accordance with Fraser’s policies and procedures. Documents care coordination activities in EMR. Maintain all records in accordance with HIPAA requirements. Accurately document services delivered and submit all billing to Accounting department within monthly requirements.

Location: Bloomington, MN / remote 

Pay: $20.00-$22.00 per hour

Schedule: This is a full-time hybrid position working during standard business hours, Monday - Friday.
