Certified HHAs - Genesee County residents need you! Starting rate is $17.55/hr. plus additional incentive pay available Summary: A Certified Home Health Aide provide personal care and supportive services in the patient's home. Essential Responsibilities: + Provide health related services as outlined in the plan of care authorized by a physician and established by a registered nurse or licensed therapist under the direction of the Aide Matrix established by the NYS Department of Health. + Provide assistance with personal hygiene, housekeeping and/or other related supportive tasks to a patient with health care needs in his/her home. + Immediately report changes in clients condition to supervising professional. + Ensure the safety of the patient, patients family and/or the employee. + Maintain HIPPA. + Complete and submit necessary patient documentation including charting tasks and observations on android device. + Attend ongoing in-service programs in a timely manner to maintain certification. + All other duties as assigned. Minimum Qualifications: + High School diploma or GED preferred. + Completion of an approved program of PCA or HHA certification as designated by NYS Dept of Health. + Must have proof of completion of HHA Training Program (Original HHA Certificate). + Must have worked for a Licensed Home Care Agency (LHCSA) within the last 24 months as documented on the NYS Home Care Registry + Valid Driver's License and reliable transportation.
