Description:The certified nurse assistant is responsible for providing various care services necessary for attending to the personal
needs, support, and health status of participants.

Location: Trinity Health PACE Montogomery County

Bilingual preferred.

Position Purpose:
Join our compassionate team at Trinity Health PACE as a Certified Nurse Assistant (CNA), where you will play a vital role in enhancing the lives of our participants through personal care, support, and safety.

What You Will Do:

Assist participants with activities of daily living, including grooming, toileting, and mobility.Monitor and report on participants' health status, noting any changes and communicating with the care team.Provide a safe and caring environment, ensuring the well-being of all participants.Support the interdisciplinary team by contributing to participant assessments and care plans.Escort participants to appointments and assist with transportation.Maintain accurate documentation of services provided and ensure confidentiality of participant information.

Minimal Qualifications:

High school diploma or equivalent.Must be a certified CNA in the state where employed.Own Vehicle and Insurance. Clean Driving Record.At least 1 year of experience working with elderly or frail populations preferred.Strong communication skills and ability to work collaboratively.Ability to work independently and manage responsibilities with minimal supervision.
