This role works under general direction of the VP of Partner Operations and as a part of the Ideal Option clinic team. The following functions have been determined by Ideal Option to be essential to the successful performance of this position which includes but is not limited to: + Performs outreach and education to eligible individuals + Performs individual patient screening to determine needs and resources and eligibility for services as needed + Collects and reports Substance Abuse and Mental Health Services Administration required survey data at intake, 6 months and 12 months + Tracks and enters demographic and related information for grant data requirement + Complete weekly and monthly reporting as needed + Facilitates ongoing coordination of follow-up care to treatment and recovery support services (e.g., transportation, housing, employment, in-patient facilities, detox, mental health and/or chemical dependency assessments, primary care, food and clothing resources) + Provides support for individual’s self-management and engagement + Work closely with clinic team to support treatment plan + Collaborate with IO peer outreach team to promote long-term recovery + Develops community partner relationships + Complies with national Protected Health Information laws + This includes HIPAA and 42 CFR Part 2 + Maintains continual sustained attendance + Other projects and duties as assigned Physical Demands: + Sedentary – up to 10 pounds Schedule Requirements: + Monday – Friday, 8am – 5pm Travel Requirements: + Occasional travel to other clinics Ouridealcandidate: Will be a certified community health worker or peer specialist. Requirements Minimum Requirements: + NAADAC certified peer counselor + Minimum of 2 years of self-attested recovery from a substance use or mental health disorder as well as 2 years of abstinence from illicit drugs and alcohol. + 1 year of working in community health or addiction treatment. + Ability to communicate effectively with patients and adapt to individual needs + Ability to communicate effectively within the organization and community agency members + Problem solving and decision-making skills + High degree of accuracy and detail orientation in data entry + Uphold the principles of ethical conduct + A knowledge of substance use disorders and their effects on individuals, families, and communities Preferred Qualifications: + Candidates may be a licensed substance use disorder professional (SUDP), social worker, community health worker, or primary healthcare worker Salary Description $22 - $24
