Summary
Initiates individual and group tobacco counseling and provides follow-up care through the cessation/treatment process. Collaborates with physicians, office practices, and other medical professionals for patient referrals. Participates in community outreach initiatives related to tobacco cessation. Schedules and triages services for the tobacco treatment program.

Job DutiesPerforms patient assessment and facilitates group and individual cessation counseling for outpatient tobacco treatment services. Documents all patient encounters. Maintains individualized treatment programs and follow-up care. Performs follow-up contact (telephone or direct mail) for inpatient tobacco cessation consults. Conducts outreach in-network and outside provider practices to establish cessation partnerships. Provides pharmacology recommendations for patients and coordinates results with primary care provider. Carries out necessary tasks for triage and data collection. Ensures that assessment and treatment are appropriate to the age, education, and culture of the patient and families.
Minimum QualificationsBachelor’s Degree in social work, counseling, respiratory care, or psychology. 2 years of experience in a clinical and educational setting and 1 year of experience with tobacco cessation program activities or related counseling (addiction). Knowledge of Microsoft Word, Access, and Excel. DL - Driver's License_PA - State of Pennsylvania Upon Hire
Preferred QualificationsMaster’s Degree in social work, counseling, respiratory care, or psychology and completion of a tobacco treatment program through one or more recognized tobacco treatment centers that are ATTUD accredited. Certified Tobacco Treatment Specialist - State of Pennsylvania Upon Hire
Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
